Appendix 4: Learning resources and templates for reflection and appraisal

All health professionals need to keep accurate learning records, which should include:

  • type of education, training and learning e.g. online learning, course attendance, group case discussion, independent learning
  • brief description of topic and why this meets your own learning needs
  • reflection, learning outcomes, things you would change or take away to incorporate into your practice. If helpful the learning record can be linked to specific competencies.

The following *templates can be used to record learning and to discuss continued professional development or learning needs at appraisal/review meetings. This in turn will evidence the appropriate level of competence for revalidation.

* The templates are based on structured questions commonly used after training events and the RCGP safeguarding templates.

Safeguarding Reflection/Case Review

This safeguarding reflection/case review structured template can be completed to demonstrate safeguarding reflection and learning across the breadth of all the areas of safeguarding children, and children and young people in care competencies.

Safeguarding Learning Event

This safeguarding learning event structured template can be completed to demonstrate safeguarding learning across the breadth of all the areas of safeguarding children, and children and young people in care competencies.

Safeguarding Reflective Practice – Annual Summary

This safeguarding reflective practice/summary structured template can be completed to demonstrate safeguarding reflection and learning across a year, related to safeguarding children, and children and young people in care competencies.
